Frequently Asked Questions

When having my loved one admitted what documents should I bring when coming to sign the admission paperwork?

Prior or to or upon admission to Dyer Nursing and Rehabilitation Center please bring the following documents:

  • Medicare Card
  • Medicaid Card or Application
  • Medicare Part D Card
  • Photo ID
  • Insurance Cards
  • Supplemental Insurance Cards
  • Long term Care Insurance Policy (If applicable)
  • Social Security Card
  • Living Will
  • Power of Attorney Documents Healthcare and/or Finances
  • Financial information including assets and income (if looking for long term placement)

What are the visiting hours?

Families and loved ones are welcome to visit 24 hours a day as this is the individual’s home while they are residing with us. We would recommend visiting from 8am – 8pm. Before and after those hours we do lock our doors and many residents are asleep.

Is it okay for children to visit?

We encourage children to visit as they are therapeutic to our elderly residents. If you are bringing children to the facility under the age of 12 please just inform the individual’s nurse.

Is it okay for a pet to visit?

We also encourage pets to visit as they are also therapeutic to our elderly residents. They are considered part of the family. Please remember to clean up after them and take proper safety measures at all times. All pets are to be kept on a leash and need to be current on all vaccinations.

What type of clothing should I bring and how much?

We recommend approximately enough clothes for between 5 – 7 days. It is recommended that you label all of your belongings. The following is a suggested list:2012-12-19-11.05

  • slacks (elastic waistband)
  • shirts (no buttons)
  • dresses
  • undergarments
  • comfortable shoes
  • gym shoes
  • grip socks
  • pajamas and/or robe
  • sweater/sweatshirt
  • coat, hat, gloves (seasonal)

Any other personal items brought should also be labeled for identification purposes.

How often will my loved one receive therapy services?

Once an individual is admitted to the facility they will be evaluated by the therapy team. Based on the evaluation we will determine how often an individual will receive therapy services.

Will I have a television in my room?

For our short-term stay rooms we have flat screen televisions with cable. We also have televisions in our lounge areas in the facility.

Are there activities going on throughout the day?

There are many different activities that are being held throughout each day. A monthly calendar is posted in each individual’s room and is posted throughout the center. Scheduled activities may include: Bingo, live entertainment, gardening, fitness activities, and games.

What role does Social Services play at Dyer Nursing and Rehabilitation Center?

Our social service team assists both the patients and their families during various times of need. They will meet with each individual from the time of admission to start getting ready for discharge planning. They can arrange professional services for: Dental, Vision, Podiatry, Audiology, Power of Attorney, Dementia Assessment, Psychosocial and Psychiatric Evaluation, and Individual Therapy.
