When nursing and rehabilitation staff work together to provide patient care, the patient is the one who benefits the most. Studies have shown that when physicians and nurses and therapists use a collaborative care model to deliver services, it results in better outcomes for patients, including chances for a shorter stay.
A team that works well together has mutual respect for everyone who is part of the team, values everyone’s contribution, and communicates effectively. The members of each different discipline must be willing to work as part of a team. They have to support the same objectives and goals, and create a plan of care that is individualized for the specific patient. It’s especially important that all team members are valued for their piece of the puzzle, and that the team is able to work smoothly toward the common goal. When that happens, the patient is the one who comes out a winner.
Communication is obviously a key to successful collaboration. Good communication among team members must be open, respectful, and purposeful. And the patient has to be included in the loop. The voice of the patient is an essential component of the system.
